Self-Insured Medical Plans

The WFCA Health Care Program offers 6 self-funded health care plans that include medical, prescription drug and vision coverage. Click on the links of the medical plans to see a 2017 Summary of Benefits for each plan.

Medical Plan

Who is Eligible?

  • Active Employees and their families
  • Qualified Volunteers and their families are eligible to enroll in the PPO-300, PPO 750 and HD medical plan
    • Responsible for monthly premiums
  • Spouses and dependents of LEOFF1 Active Employees
  • Early retirees and individuals who have not reached the Medicare eligible age
    • Responsible for monthly premiums
  • If enrolled in the Plan, you may also enroll dependents and spouses
    • Must enroll in the Retiree Plan upon reaching Medicare eligibility
  • Individuals retiring who have reached the Medicare eligible age
    • Must enroll in the Retiree Plan at the time of retirement
    • Responsible for monthly premiums
    • If enrolled in the Plan, you may also enroll dependents and spouses
  • LEOFF 1 Active Employees and Retirees
  • Dependents of LEOFF 1 actives and retirees

Wellness Portal

Member fire service organizations and their employees enrolled in a self-funded health plan have full access to a Wellness Portal at no cost. Fire service organizations with volunteers may provide access to the Wellness Portal for a nominal per person per month fee.
